Parental Agreement

This agreement outlines the guidelines for working with Health Coach and Therapist Shannon Sciametta. By signing, you acknowledge that you understand and agree to these terms and that your participation is completely voluntary.

Shannon provides guidance, support, and therapeutic interventions, but does not diagnose, prescribe, or replace medical or mental health care from your child’s healthcare provider or pediatrician.

All information and recommendations are customized for your child and intended for your use only. Please do not share publicly or on social media.

Emails are for clarifying questions about your child’s current plan. Questions should be brief; longer discussions may require a separate appointment.

Shannon may not be available on weekends, holidays, or during vacation periods.
